110-300-0210(1)(2) Licensor reviewed child files and observed CIS forms were not current, complete and medically verified. 110-300-0210(3) Licensor reviewed child files and observed no parent or guardian’s written proof the child is scheduled to be immunized or a signed and dated statement detailing when the child’s immunizations will be brough up to date and stating they understand their child will be excluded from care if the immunizations are no completed within 30 day of the specified due date. 110-300-0210(5) Licensor reviewed child files and observed no written proof that the child’s family, case worker or health care provider are in the process of obtaining the child’s immunizations records.
